Known issue: after installing, the effect may not show up under Desktop Effects even though it's actually loaded. Check with:
$ qdbus org.kde.KWin /Effects org.kde.kwin.Effects.listOfEffectsIf it shows up there but not in the UI, it's a KWin plugin version (IID) mismatch — the plugin was built against a different KWin version than the one installed. Fix it with a full system update and reboot:
$ sudo dnf update

This prevents it from conflicting with Better Blur DX (running both at once silently breaks refraction):
$ kwriteconfig6 --file kwinrc --group Plugins --key blurEnabled false
Refraction only shows up if the panel background is fully Transparent — "Adaptive" isn't enough, since it needs something behind it to distort at all times.
Right-click panel → Edit Panel → Panel background opacity → Transparent
System Settings → Window Management → Desktop Effects → Better Blur DX
You can do this on your own, it's pretty easy, but I'll leave my config here anyway
Heads up: Refraction Strength defaults to 0. If your panel just looks like plain blur with no distortion after enabling the effect, this is why — turn it up manually.
For compiled KWin effects like this one, a simple "Apply" isn't always enough — log out and back in to make sure the new values are actually loaded.
